Are you a credentialed pharmacist passionate about making a positive impact on patient health outcomes, particularly in aged care patients and people with disabilities?

We are currently seeking expressions of interest from dedicated pharmacists to join our team in performing medication reviews.

**Responsibilities:**
– Conduct comprehensive medication reviews, understanding the unique needs and challenges faced by the patients.
– Identify any potential medication-related issues and provide recommendations for optimization, taking into account factors such as polypharmacy, deprescribing, cognitive impairment, and functional limitations.
– Collaborate closely with healthcare professionals and caregivers to ensure integrated and holistic patient care.
– Prepare clinical reports summarizing medication review findings and recommendations for the GP, facilitating communication and coordination of care.
– Provide medication lists and counseling to patients and caregivers, ensuring understanding of medication regimens and promoting medication adherence.
– Document medication reviews accurately and sensitively, maintaining patient confidentiality and dignity at all times.

Requirements:
– Degree in Pharmacy
– Registration with AHPRA
– Relevant clinical credentials
– Strong understanding of medication management, with a focus on geriatrics and disability care.
– Empathy, patience, and compassion in working with elders and patients with disabilities.
– Excellent communication and interpersonal skills, with the ability to communicate effectively with patients, caregivers, and healthcare professionals.
